The UK is to publish reports into serious errors in fertility clinics to ensure greater transparency, the Human Fertilisation and Embryology Authority (HFEA) has announced.

In 2007–08, 182 mistakes were reported by clinics from a total of nearly 52,000 cycles of in vitro fertilisation treatment, the HFEA said.

There were eight grade A incidents, the most serious errors, which included two mix ups where the wrong embryo or the wrong gametes had been used.

The new policy will apply to incidents taking place on or after 1 October 2009.
