Memorial Health Care System, a not-for-profit, faith-based healthcare organisation, has selected Aegis Health Group to assist in expanding its population health management programmes.
Aegis’ population health management (PHM) solution helps providers extend their outreach to positively impact health and wellness across a wider community population.
The new initiative will focus on partnerships with local employers to improve the health of their workforces, which has demonstrated success in lowering health-related costs while supporting the health system’s mission of creating healthier communities.
Memorial Health will offer employers a resource called OneCommunity, an online population health portal designed to identify and address employees’ hidden health risks before they turn into a crisis.
This new population health portal allows consumers to record and track health information online.
Memorial Health Care System vice president Andrew McGill said that helping people live healthier, more fulfilling lives has always been a central part of Memorial Health Care System’s outreach efforts.
"Our goal is to more proactively help local employers identify and address workforce health risks and put employees on a path to better health through targeted interaction, education and intervention," McGill added.
Aegis’ tools, currently used by leading health systems across the country, are expected to help Memorial match at-risk individuals with appropriate programmes and services offered by the health system.
Healthcare reform has emphasised the importance of worksite wellness programmes. In addition to this, the law also has increased the value of incentives employers can offer to encourage worker participation.
Aegis Health Group executive vice president of operations Yale Miller said that employers and health systems share a common goal – improving the health of the community and helping organisations lower overall healthcare costs.
"As the area’s healthcare leader, Memorial Health Care is perfectly positioned to take the reins in accomplishing this goal," Miller added.
Memorial Health Care System is a not-for-profit, regional referral centre with 4,000 associates and more than 700 affiliated physicians providing healthcare throughout south-east Tennessee and north Georgia.
It offers a continuum of care including preventative, primary and acute hospital care as well as cancer and cardiac care, orthopaedic and rehabilitation services.
