Crystal Run Healthcare in New York, US, has partnered with AMC Health to enhance patient care outcomes.
The healthcare group will leverage AMC Health’s technology, which is designed to enhance care for patients suffering from heart failure, diabetes, chronic obstructive pulmonary disease and other chronic illnesses.
Crystal Run Healthcare co-chief clinical transformation officer Jonathan Nasser said AMC Health’s telehealth solutions reduce unnecessary resource utilisation, particularly preventable 30-day readmissions.
"By identifying care gaps and providing services and tools that support seamless and effective care transitions, AMC Health will make it easier for our doctors to deliver high-quality, cost-effective and efficient evidence-based care," Nasser added.
"This will position us to successfully navigate the shift to an outcomes-based reimbursement system."
The multi-speciality group practice, with over 300 providers across 15 practice locations, will utilise remote patient monitoring services to indentify and eliminate care gaps that can occur after a patient’s discharge from hospital.
AMC Health will provide a post-hospital discharge service, as well as tailored disease-specific programmes that include biometric monitoring, healthcare coaching and logistical services for in-home monitoring.
AMC Health president and CEO Nesim Bildirici said the company’s comprehensive telehealth offerings will facilitate access to clinically actionable data, enabling Crystal Run’s clinicians to detect pre-acute conditions earlier and keep patients out of the hospital.
