Oklahoma healthcare system to revamp population health management strategy

Hillcrest HealthCare System (HHS), a network of six hospitals and 49 clinics in north-east Oklahoma, US, has chosen Phytel to aid in the execution of its population health management strategy.

Hillcrest HealthCare System (HHS), a network of six hospitals and 49 clinics in north-east Oklahoma, US, has chosen Phytel to aid in the execution of its population health management strategy.

HHS will deploy Phytel Outreach to primary care physicians to provide recommended preventive and chronic care, and also plans to implement Phytel Insight.

Utica Park Clinic chief medical officer Jeffrey Galles said the clinic will use Phytel’s solutions to help build the infrastructure around the CPCI programme and other quality monitoring programmes.

"We can also use these tools to build additional infrastructure if we elect to move forward with an accountable care organisation. Phytel’s technology provides an ideal fit for us now and into the future," Galles added.

Phytel Outreach allows the physician to identify patients due for recommended care, automatically notifies patients through automated messaging, and tracks patient response and monitors compliance.

"Physician-led population health improvement is crucial to making healthcare more affordable, effective and patient-centred."

Phytel Insight leverages a web-based registry and analytics to measure the effectiveness of quality initiatives and generate actionable integrated data to improve health outcomes across a population, according to Phytel.

Phytel Insight can identify patients whose care falls outside established guidelines, assess individual provider and practice performance across the healthcare enterprise, and demonstrate quality improvements to achieve patient-centered medical home, meaningful use and P4P qualification.

Phytel CEO Steve Schelhammer said that Hillcrest HealthCare System is embarking on a bold initiative to improve the quality of care for its patients.
