The New Jersey Hospital Association’s Institute for Quality and Patient Safety has launched a one-year collaboration to reduce hospital readmissions due to heart failure and improve the quality of healthcare throughout the US state.
More than 50 organisations are participating in the initiative, representing hospitals, nursing homes, home health and hospices.
Facility team members and industry leaders will meet for a series of learning sessions over the next year, where they can also share data, best practices and strategies.
The goals of the collaboration include gaining a better understanding of why patients are being readmitted to hospitals, identifying best practices and developing resources for hospitals and post-acute providers to improve healthcare for heart failure patients.
