The solution integrates dedicated care navigators, intelligent patient care coordination, and curated provider networks, using AI to reduce avoidable costs and close care gaps. It is designed to support fee-for-service (FFS) and value-based care (VBC) models.

Bamboo Bridge extends navigation capacity without the need for additional staff. It enables providers and health plans to identify and engage high-need patients using predictive modelling, real-time care intelligence, and AI-driven analytics.
The system navigates patients from primary care referrals, post-discharge, and emergency departments to appropriate in-network providers, aiming to reduce avoidable emergency department and inpatient use.
It integrates into current workflows with optional prescription drug monitoring programme (PDMP)-enabled support, electronic health record (EHR)-embedded navigation, and admission, discharge, and transfer (ADT)-based activation.
Additionally, Bamboo Bridge is designed to deliver both financial and clinical results for FFS and VBC arrangements, including improvements in network placement, healthcare effectiveness data and information set (HEDIS) measures, and risk coding.
