CharmHealth and PointClickCare have announced a partnership to enable real-time, bidirectional post-acute care data exchange between their platforms.
CharmHealth’s electronic health record (EHR) system will be integrated with PointClickCare’s platform for long-term post-acute care (LTPAC) facilities, aiming to automate information flow and enhance coordination among healthcare providers.

The integration is aimed at supporting LTPAC facilities that face persistent challenges with interoperability, often resulting in gaps in health data exchange with clinicians, hospitals, payers and other partners.
These data silos can lead to rework, delayed authorisations, billing denials, and preventable readmissions. The new integration seeks to address these challenges by allowing instant syncing of patient data across systems.
The integration features automated patient data synchronisation, eliminating manual entry by automatically syncing diagnoses, demographics, medications, allergies, and insurance information from PointClickCare into CharmHealth. This ensures record consistency across both platforms.
Additionally, when providers sign chart notes in CharmHealth, these are instantly transmitted back to PointClickCare as progress notes or documents. The alignment keeps both outpatient and long-term care teams updated on patient status.
