With Skilled, Sage Core now provides a single care platform supporting skilled nursing, assisted living, independent living, and memory care.

Sage Core connects with electronic health records (EHR) and features underwriters laboratories (UL) 1069 certified system for high-acuity workflows.

The solution integrates a real-time insight engine for resident care data, documentation workflows intended to enhance minimum data set (MDS) accuracy, and a wireless-first nurse call system, to enable earlier detection of condition changes.

With Sage Skilled, operators will be able to enhance reimbursement processes, decrease preventable hospital transfers, and strengthen clinical accuracy, while offering reduced installation expenses.

Early controlled deployments of Skilled are said to have shown measurable impacts.

A continuing care retirement community and skilled nursing rehabilitation centre maintained response times of under four minutes and recorded a reduction in 30-day re-hospitalisations by nearly 50% year on year.

These outcomes were supported by quicker certified nursing assistant (CNA) responses, proactive escalation measures, and staffing decisions informed by data.
