CenterLight is the not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the US. Its services are delivered through 11 PACE centres, one owned alternative care setting, and a contracted healthcare providers network in the area.
Stellar Health’s platform is designed to simplify quality incentive programmes by translating organisational goals into actionable care tasks that fit into the providers’ daily workflows.
It also offers transparent and timely financial incentives, distributed monthly and potentially shared by the entire care team. This model is intended to foster provider engagement while driving improvements in population-based patient outcomes.
