The Albany Med Health System in the US has partnered with physician-led Honest Health to improve patient care.

The collaboration marks the extension of Honest Health's existing partnerships from Western New York to the Capital Region.

It will provide healthcare professionals with tools and support to improve patient outcomes and drive cost efficiencies.

Honest Health, as part of this partnership, will offer its expertise in value-based care to Albany Med's physicians, practice providers and nurses.

They will gain access to predictive analytics, real-time insights and population health management strategies and will be able to concentrate on delivering quality care.

Albany Med Health System president and CEO Dennis McKenna said: "The Albany Med Health System is committed to delivering the highest quality of care to the communities we serve.

The partnership will focus on traditional Medicare patients initially, with a collaborative effort on clinical and financial accountability.

Both organisations will deliver measurable improvements in care by utilising streamlined processes, technology and a joint approach, with a priority on value-based care.
