Commence and The Language Group (TLG) have introduced the Fetch solution that provides AI-driven translations of patient discharge instructions reviewed by humans, directly into electronic health record (EHR) systems in real time.
Built for swift scaling across commercial healthcare settings, the tool seeks to support patients with limited English proficiency (LEP) by providing clear, accurate instructions in their preferred language at the point of discharge, enhancing understanding, reducing readmissions and helping to meet statutory requirements.

Fetch automates translation within standard discharge workflows so clinicians can provide understandable instructions before a patient leaves the room, replacing fragmented, manual processes that often take hours or days.
A large integrated health system that serves 150-200 LEP patients daily estimated readmissions at 14% for patients discharged without translated instructions, at an average cost of $15,200 per readmission.
The health system calculated potential annual savings of more than $4.6m with Fetch for a system of this size.
The translation solution delivers discharge instructions in minutes and provides unified dashboards for compliance, utilisation and cost tracking.
Fetch integrates with major EHR platforms, including Epic, and is approved and listed in the Epic Showroom.
The solution is designed to meet standards for patient safety and regulatory compliance, including Section 1557 of the Affordable Care Act, Title VI, CLAS standards, Joint Commission requirements, HIPAA, SOC 2 Type II and ISO 18587 certification.
By embedding into existing workflows, the system targets delays, coverage gaps and risks across diverse patient populations.
