Memorial Healthcare launches diabetes medication management initiative

The initiative uses DrFirst's prescription fill data to identify patients at risk of non-adherence and polypharmacy.

US-based Memorial Healthcare System has launched a diabetes medication management programme to improve medication adherence and health outcomes for patients.

This initiative uses DrFirst's prescription fill data to identify patients at risk of non-adherence and polypharmacy, allowing early intervention for therapy optimisation and reducing care gaps.

The programme targets more than 1,100 diabetes patients using DrFirst's population health management solution.

Pharmacy care teams will engage the identified at-risk patients via digital tools and telehealth to support timely clinical interventions.

This initiative builds on Memorial Healthcare System's ten years of experience with DrFirst's medication history solution, which has facilitated safer and informed prescribing.

Memorial Healthcare System's ambulatory and population health pharmacy services director Dovena Lazaridis said: “By developing programmes that serve high-risk patients, we shift from reactive to proactive interdisciplinary care, improving access, optimising therapy, and supporting adherence, driving meaningful outcomes and building a healthier community.

“By using timely, comprehensive medication history data, our care teams can proactively identify adherence barriers, intervene early, and help prevent complications that often result in hospitalisations, ultimately improving care while reducing avoidable costs.” 

Now integrated into Memorial Healthcare System's Epic electronic health record, DrFirst's solution provides prescription fill data in real time.

This integration, at the point of care, promotes informed decision-making and efficient workflows.

Over the coming months, the pharmacy team will assess the effect of their tailored patient support and share their findings through research efforts in the future.

DrFirst CEO Laizer Kornwasser said: “Timing and precision are essential for successful adherence programmes. When you can identify which patients are drifting off course with their medications and intervene before complications arise, you transition from reactive to proactive care.

“Memorial Healthcare System’s Pharmacy Population Health services explore what’s possible when you combine intelligent data with clinical intent.” 

In the future, Memorial Healthcare System intends to extend its Population Health Pharmacy services to manage other conditions, including high cholesterol, high blood pressure, and COPD, building on the foundation established by its diabetes-focused initiative.
