St. Luke’s University Health Network extends Care Management agreement with CarePort

St. Luke's University Health Network (UHN) has extended its Care Management agreement with CarePort to now include additional solutions across its network in Pennsylvania and New Jersey.

St. Luke’s University Health Network (UHN) has extended its Care Management agreement with CarePort to now include additional solutions across its network in Pennsylvania and New Jersey.

With this extended deal, the US-based care management and post-acute outcomes solutions provider CarePort will secure a new market for CarePort Guide, Connect and Insight.

CarePort Guide, Connect, and Insight solutions will enable UHN to engage patients and their families in post-acute care selection process besides being able to precisely monitor at-risk patients across the continuum and manage the post-acute network.

With the web-based solutions, providers will be able to coordinate care through information sharing in real time, automated data capture and analytics.

St. Luke’s University Hospital IP director for care management Marlee Wilson said: “CarePort offers the advanced technology care managers need to meet the needs of our patients and their caregivers.

“Accelerated access to view detailed next level of care options creates a more efficient and seamless transitional care process for clinicians and patients alike.”

With the inclusion  of CarePort Guide, Connect and Insight to St. Luke’s Care Management solution portfolio, patient care is expected to be enhanced due to one cohesive platform across St. Luke’s locations. Clinicians at St. Luke’s will get visibility into patient movement in real time throughout the continuum – right from the time patients get admitted to their post-discharge period.

St. Luke’s University Health Network is a non-profit, integrated network offering services at ten hospitals and over 315 locations in several counties of Pennsylvania and New Jersey.

In April,  the Allscripts firm CarePort Health began managing the Allscripts Care Management solution and services portfolio. This move was intended to speed up innovation of the Care Management platform. With the Care Management and CarePort platforms, providers, payers and ACOs can gain visibility across all levels of care and transitions across the continuum.
