
US-based not-for-profit health care organisation John Muir Health has expanded its partnership with CarePort by investing in its solutions CarePort Connect and CarePort Insight to manage patients.
The investment in CarePort solutions is aimed at better tracking and managing patients as they travel across the continuum including hospitals, ACOs, physician groups, payers and post-acute providers.
CarePort stated that Connect enables disparate providers that share patients across the continuum to coordinate care through real-time, actionable alerts and data.
Through this, the company is aiming to reduce hospital readmissions, enhance patient outcomes and optimise care transitions.
John Muir Health Care Management, and Continuum Services Population Health executive director Ann Orders said: “With cross-continuum data sharing enabled by CarePort, we’ll be able to better manage patients from admission through post-acute discharge.
“Readmission reduction is a key focus area for us, and we are excited to leverage CarePort’s technology to enhance these outcomes.”

By GlobalDataCarePort Insight will be utilised to optimise post-acute network management and work more collaboratively with its SNF partners.
John Muir Health will measure and benchmark performance within its SNF network using real-time data and referring source metrics from Insight.
It will help to better track hospital readmissions, detect potential gaps in care, and identify opportunities for improvement.
