ABOUT Healthcare and Panda Health have joined forces to launch a new Care Transition Management (CTM) category in the digital health marketplace.

This new category will include solutions that help hospitals and health systems optimise the transition of care for patients as they move from hospitals to post-acute care settings.

Additionally, CTM solutions are said to help hospitals and health systems to lower readmission rates and healthcare expenses.
