Bamboo Health has announced a partnership with Radial to improve care collaboration and value-based outcomes.

This collaboration aims to provide clinicians with actionable insights to streamline care transitions and cut healthcare costs.

Bamboo Health's Pings solution will be integrated with Radial's Care software platform, offering providers real-time care intelligence.

This synergy is expected to empower healthcare providers by delivering member-specific insights, advanced risk stratification, and enabling targeted interventions at the optimal time.

The combined capabilities of both platforms will allow for instant notifications on patient admissions or discharges, identification of patients for home-based post-acute care, and timely referrals to palliative or hospice care when necessary.

The collaboration is particularly significant for providers involved in accountable care, where managing the total cost of care is crucial.

Shared savings providers serving Medicare patients, who often have complex health needs, will benefit from this partnership.

By directing patients to suitable care programmes, such as palliative care or hospital-at-home initiatives, prolonged hospital stays can be reduced, care efficiency enhanced, and patients' quality of life will be improved.
