Clarity Health Care is set to expand with a new multidisciplinary mental health clinic opening in Malvern, Victoria, Australia, bringing a range of professionals together to offer centralised care plans for clients.

The model is designed to coordinate treatment and improve continuity for patients in a single location.

The organisation has previously established clinics in Fitzroy and Hobart, where the integrated care model has significantly reduced readmission rates to 18.6% from an average of 48%.

The new clinic is expected to extend these benefits to the inner south-east and surrounding areas.

Clarity Health Care CEO Davis Lemke said: “Clarity is built on the belief that mental health care works best when it's connected, compassionate, and collaborative.

The clinic, which will accept both self-referrals and GP referrals, is designed to cater to individuals and families throughout their mental health journey.

It will offer flexible options for appointments, underscoring the company's focus on accessible and continuous care provision.

The clinic's environment has also been crafted to facilitate a therapeutic experience for both clinicians and clients.

Since its founding in 2013, Clarity Health Care has been providing alternatives to hospital-based treatment for those with mental illness.

The organisation delivers care across Melbourne and Hobart through a team of professionals, including psychologists, psychiatrists, mental health social workers, occupational therapists and nurses.
