Cleveland Clinic ACO launches dementia care pilot initiative

The tie-up is part of the Cleveland Clinic ACO's focus on enabling patients to age independently at home for as long as feasible.

The Cleveland Clinic Medicare Accountable Care Organization (ACO) in the US has teamed up with virtual solutions provider Remo Health to introduce a pilot initiative linked to dementia care.

The alliance is aimed at offering enhanced support to patients with dementia and their care providers in Ohio, US.

It is part of the Cleveland Clinic ACO's focus on enabling patients to age independently at home for as long as feasible.

The initiative is designed to address the rising requirements of those suffering from the health condition by offering tailored care and resources.

This is particularly vital for rural families who may travel long distances for doctor visits, as well as working care professionals.

Patients under the care of the Cleveland Clinic ACO in Ohio will also benefit from joining a national support network that encompasses 20,000 families, offering a platform to connect with others facing similar challenges.

Cleveland Clinic ACO president Jessica Hohman said: “As dementia becomes increasingly prevalent, it will place significant pressure on our health care system to meet the complex and continuous needs of patients and their caregivers.

“We’re excited to embark on this collaboration to deliver wraparound care and support that extends beyond the clinical setting and into the home. It’s important we continue to search for innovative solutions to help guide patients and families through every stage of the 24/7 dementia journey.”

In Ohio, around a quarter of a million individuals are suffering from dementia, and more than 450,000 serve as unpaid caregivers, often experiencing isolation and overwhelming responsibilities.

Two-thirds of these care providers also contend with their chronic health conditions, which are frequently exacerbated by the stress and demands of caregiving.
