Findhelp and AIDA Healthcare have formed a partnership for post-acute care referral integration, aiming to streamline and improve acute to post-acute care setting transition while addressing the social drivers of health (SDoH) of patients.
The collaboration combines Findhelp’s social care services network with AIDA Healthcare’s post-acute referral workflows, supporting healthcare providers, systems, and payers in optimising patient transitions and reducing administrative workload.
It enables seamless referral workflows to a post-acute care providers network, including hospice agencies, skilled nursing facilities, and home health agencies.
AIDA’s referral solution integration within the Findhelp platform automates care transitions and information sharing for healthcare teams.
The joint network also supports closed loop referrals, allowing for workflows from discharge to acceptance with fewer manual processes and transparency.
Additionally, the partnership prioritises reducing inpatient length of stay (LOS) and matching patients with appropriate post-acute care providers to lower costs and enhance patient experience.
It also seeks to improve coordination, ensure interoperability, and maintain data security among acute and post-acute care, social support services, and community-based providers.
By integrating Findhelp's platform with AIDA Healthcare's referral orchestration offerings, providers and health systems can benefit from better alignment of provider availability and patient preferences, faster referral acceptance, enhanced data reporting, and reduced administrative work.
