The Advanced Care Primary Care (ACPC) Clinic by HealthPartners coordinates healthcare, helping patients with intricate requirements.
Early results indicate a 60% reduction in avoidable emergency visits for those who visited the most in the previous year.

The ACPC Clinic model integrates a multidisciplinary team to manage the entire care journey of patients.
Daily meetings are held to discuss recent and upcoming patient visits, ensuring a coordinated response to any emergency department visit or hospitalisation. The team's approach aims to provide the right next steps for patient support quickly.
Primary care clinicians refer patients to the clinic, frequently on account of existing medical issues or difficulties like insufficient social assistance or food insecurity.
The clinic employs schedulers to assist with appointment setting and clinic-support teams to address barriers to care.
Additionally, video visits and telephonic help are offered to facilitate convenient access to care.
The ACPC Clinic provides a comprehensive approach to care coordination, encompassing primary and speciality care, pharmacy, home care, palliative care, and behavioural health.
The team at the clinic offers timely follow-up and close monitoring.
Improved care coordination efforts also encompass facilitating transitions between care settings and collaborating with health plan care coordinators to maximise patient benefits and services.
The ACPC Clinic serves patients enrolled in specific health plans such as HealthPartners Minnesota Senior Health Options, HealthPartners Medicare Advantage, Blue Cross Blue Shield commercial plans, and individuals under Medicare’s ACO REACH programme.
