Digital advance care planning (ACP) solutions provider MyDirectives will integrate its MyDirectives for Clinicians (MyDC) platform directly into the KanTime Healthcare Software’s electronic health record (EHR) system.
This collaboration aims to streamline access to patients' advance care plans for home health, palliative, and hospice providers, enhancing patient-centred care and avoiding unnecessary interventions.
The integration is currently underway, and complete availability is anticipated shortly. It will enable KanTime's clients to retrieve and share essential ACP documents within their existing workflow.
These documents include POLST forms, advance directives, Five Wishes, and mental health advance directives (MHADs).
This solution helps healthcare professionals to honour patient wishes across care settings by ensuring that the relevant documents are easily accessible, a crucial factor in emergency and transitional care scenarios.
Integration activation is designed to be simple, with the majority of the preparatory work already completed.
KanTime clients can easily enable access to MyDC by contacting the company for their MyDC licence activation.
The collaboration also provides KanTime users with the advantage of accessing ACP documents anywhere and anytime through the A|D Vault Exchange registry, which is managed nationwide by MyDirectives.
The A|D Vault Exchange is developed using a platform that is HITRUST-certified and has undergone an SOC 2 Type II audit, ensuring important documents can be shared and accessed securely across healthcare organisations, health information exchanges (HIEs), and in emergency contexts.
