The programme targets more than 1,100 diabetes patients using DrFirst’s population health management solution.

Pharmacy care teams will engage the identified at-risk patients via digital tools and telehealth to support timely clinical interventions.

This initiative builds on Memorial Healthcare System’s ten years of experience with DrFirst’s medication history solution, which has facilitated safer and informed prescribing.
