These documents include POLST forms, advance directives, Five Wishes, and mental health advance directives (MHADs).
This solution helps healthcare professionals to honour patient wishes across care settings by ensuring that the relevant documents are easily accessible, a crucial factor in emergency and transitional care scenarios.
