Healthcare information technology solutions provider Allscripts has completed the previously announced sale of its CarePort Health business to global health and community care technology company WellSky for $1.35bn.

The solutions offered by CarePort Health assist hospitals and post-acute care providers to coordinate and transition patients through different settings of care.

The move comes after WellSky signed a definitive agreement with Allscripts in October last year to acquire CarePort Health, a care coordination software company that connects acute and post-acute providers and payers.

It currently serves 1,000 hospitals and health systems, as well as 110,000 post-acute provider locations, Home Health Care News reported.

The acquisition of CarePort Health enables WellSky to manage the acute care discharge process, track patients across post-acute care settings.

Furthermore, WellSky will be able to apply patient and population level analytics, and support EMR-based care protocols.

Under the terms of the agreement, the customers and more than 200 employees of CarePort Health will become part of WellSky.

The EHR-agnostic suite of solutions of the business connect the discharge process with post-discharge care coordination.

Together, WellSky and CarePort Health will provide end-to-end visibility across the continuum and can improve outcomes, lower costs, and increase patient satisfaction.

Trough the combination, CarePort Health clients will obtain access to a wide network of post-acute providers and can leverage the predictive analytics suite and value-based care technologies of WellSky.

Additionally, health systems, payers, and post-acute providers can leverage these combined capabilities to effectively collaborate in a data-driven way and improve patient outcomes.
