South Carolina-based healthcare organisation Prisma Health has expanded its alliance with health-tech company Bamboo Health to offer real-time patient intelligence.
This integration aims to facilitate seamless transitions for patients across different care settings.

As part of the partnership, Prisma Health will incorporate Bamboo Health’s three solutions, including Pings, Spotlights, and Discharge Summaries, into its network, which includes more than 5,500 clinicians, 20 hospitals, and 835 practice locations.
Bamboo Health said that this suite of solutions is designed to improve care coordination and support value-based care programmes, providing real-time care collaboration at low cost.
The first solution, Pings, will provide real-time alerts for patient care transitions, utilising admission, discharge, and transfer (ADT) data, further assisting healthcare professionals in delivering enhanced care and achieving success in value-based care programmes.

According to Bamboo Health, the absence of real-time patient information can negatively impact the healthcare system, leading to missed opportunities in care coordination and quality improvement.
Better information sharing could prevent many of these readmissions.
The company cited that in South Carolina, nearly 14.49% of hospital discharges result in unplanned readmissions within 30 days, a figure close to the national average.
Besides, the new solutions will complement Bamboo Health’s existing PMP Gateway solution used by Prisma Health, which integrates multi-state prescription drug monitoring program intelligence into clinical workflows.
The expanded suite of solutions is expected to streamline processes and improve access to critical patient information, enhancing overall patient care.
