US-based remote healthcare technology platform collaboratehealth has teamed up with sleep centre management services provider MedBridge Healthcare to strengthen its Hospital to Home programme.

Through this collaboration, collaboratehealth will leverage MedBridge’s comprehensive sleep diagnostic services into its remote care platform to provide sleep apnoea screening, diagnosis, and therapy monitoring.

With this partnership, collaboratehealth aims to reduce readmission rates among patients with sleep disorders by facilitating early engagement, expedited diagnosis, follow-up treatment, and health coaching upon discharge from the hospital.

By integrating MedBridge’s sleep diagnostic services into its remote care platform, collaboratehealth seeks to further enhance patient outcomes and experiences while reducing costly and unnecessary 30-day readmissions.

This strategic partnership combines the expertise of both companies to deliver comprehensive care and support to patients with sleep disorders and ensure a seamless transition from hospital to home.
