US-based Centegra Health & Wellness Network (CHWN) has selected healthcare solutions provider Valence Health to provide infrastructure and support for its clinically integrated network (CIN).
Valence will provide CHWN with the Valence Health Vision technology solution. The solution has been designed to combine data from CHWN physician members’ practice management and electronic medical records systems, together with data from hospital information systems, lab systems, prescription drug information and other data sources, to provide a comprehensive clinical view of patient care.
Healthcare providers and coordinators can identify patient populations based on combinations of clinical information such as conditions, gaps in care, lab results and more.
Valence will also provide management services for CHWN and perform claims administration, eligibility maintenance, utilisation management and quality improvement projects to Blue Cross Blue Shield HMO members.
CHWN is looking to improve the quality and efficiency of healthcare delivered to patients served by Centegra-owned hospitals, employed physicians and affiliated healthcare providers.
CHWN clinical integration and payer strategies senior vice president Jeff Schmidt said that the partnership would deliver better population health management services and programmes to patients at affordable costs.
