BAYADA Home Health Care has introduced the Enhanced Quality of Care Model (EQoC), an AI-supported approach designed to improve safety and health outcomes for older adults living at home.
The company has launched its model for all private-pay personal care clients, combining daily nurse oversight with predictive technology to identify risk factors early and support preventative interventions.
Falls continue to be the primary cause of injury among individuals aged more than 65, affecting one in four seniors each year and often resulting in readmissions and mortality.
BAYADA presents the model as a solution to the requirements of care managers and senior living facilities for tools that ensure safety and health, prevent hospitalisation of older adults, and facilitate transparent communication and outcomes reporting, all while prioritising the delivery of appropriate care at the right moment.
The company is partnering with stakeholder organisations and researchers, including the Home Care Association of America, to enhance industry practices.
It aims to provide nurse-led care teams with trend analysis tools to improve daily monitoring and risk prevention, pinpoint predictors of adverse events and create individual risk profiles and set up industry metrics to manage client outcomes.
The EQoC model includes more than 40 daily monitoring indicators, including wounds, pain levels, mental health status, hospital admissions, and falls.
This is supported by the supervision of registered nurses, daily evaluations of trends, and continuous adjustments to care plans.
The model employs AI-generated insights to identify minor changes and take action before risks intensify, while also tailoring visit frequency to meet the needs of clients and minimise adverse events.
