Galacxia has launched an updated version of its Life Backup Plan Interoperable Lifecare Platform, featuring a standardised digital health patient workflow.

The updates enable individuals, families, clinicians, hospitals, employers, health insurers and caregivers to collect lifestyle and medical information, improving safety and strengthening decision-making across the complete care continuum.

The platform organises medical records, surgical and family history, medications, symptoms and allergies.

Life Backup Plan integrates with electronic health record (EHR) systems, decreasing the workload for healthcare providers and making it easier to access patient information.

Hospitals will also benefit from reduced information gaps and comprehensive post-discharge monitoring.  

The Life Backup Plan assists organisations in enhancing care coordination, reducing readmissions, improving Joint Commission on Accreditation of Healthcare Organizations (JCAHO) performance, and lowering Centers for Medicare & Medicaid Services penalties related to preventable readmissions.

The platform minimises employee distraction and absenteeism by recognising and responding to illnesses and emergencies at an earlier stage.

It assists workers who care for elderly parents or young children, leading to enhanced productivity and fewer health-related interruptions.
