NorthShore – Edward-Elmhurst Health partners with Lumeris

Lumeris' population health data platform will be integrated into health system’s clinically integrated network.

US-based healthcare network NorthShore – Edward-Elmhurst Health (NS-EEH) has partnered with Lumeris, a technology-powered care enablement company, to enhance coordinated care and reduce healthcare costs.

The collaboration will integrate Lumeris' population health data platform into NS-EEH's clinically integrated network, which includes more than 3,000 physicians and nine hospitals in the Chicago metropolitan area.

It supports healthcare providers in care management, pharmacy management and patient engagement.

The partners will also focus on reducing healthcare disparities in underserved communities by creating new accountable care organisation (ACO) models.

They initially plan to participate in the Centers for Medicare & Medicaid Services' ACO REACH model, which aims to improve care coordination and health outcomes for traditional Medicare patients.

In the future, NS-EEH and Lumeris intend to expand their focus to include other types of accountable care and population health models.

Lumeris chief revenue officer John Fryer said: “As the adoption of value-based care continues to increase across the country, this partnership accelerates NorthShore – Edward-Elmhurst Health’s ability to provide high-value, consumer-centric healthcare to all members of their communities.

NorthShore – Edward-Elmhurst Health operates more than 300 ambulatory locations and eight hospitals.

The group serves more than 4.2 million residents in six north-east Illinois counties, with 7,100 physicians and more than 27,000 team members.

It also provides mental health services through Linden Oaks Behavioral Health in Naperville.

Based in Missouri, Lumeris specialises in helping health systems and physician practices in value-based care by providing technology and insurance capabilities.

The company has more than $12bn of capital under management across more than 12 markets.
