Stellar Health and CenterLight team up for senior care in New York, US

Stellar Health’s platform is designed to simplify incentive programmes by translating goals into actionable care tasks.

Stellar Health and CenterLight Healthcare have announced a partnership aimed at improving provider engagement and health outcomes for seniors in New York, US.

The collaboration leverages Stellar Health’s point-of-care technology platform to support CenterLight’s network of providers and its senior members across New York.

CenterLight is the not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the US. Its services are delivered through 11 PACE centres, one owned alternative care setting, and a contracted healthcare providers network in the area.

Stellar Health’s platform is designed to simplify quality incentive programmes by translating organisational goals into actionable care tasks that fit into the providers' daily workflows.

It also offers transparent and timely financial incentives, distributed monthly and potentially shared by the entire care team. This model is intended to foster provider engagement while driving improvements in population-based patient outcomes.

Stellar Health chief revenue officer Chad Turner said: “Our partnership with CenterLight Healthcare PACE underscores our commitment to enabling and empowering providers everywhere.

“Together, we're ensuring providers can focus on what matters most: delivering high-quality care to seniors across CenterLight's PACE network.”

The collaboration is expected to expand, building on Stellar's established presence in New York.

CenterLight Healthcare president and CEO Tara Buonocore-Rut said: “We are thrilled to partner with Stellar Health to enable our providers to bring services to even more New Yorkers who may benefit from all-inclusive care.”

Stellar Health’s network includes more than 20,000 providers and 2,200 medical practices, catering to over 1.2 million patients.

CenterLight Healthcare PACE serves adults aged 55 and older throughout New York City, Westchester, Nassau, and western Suffolk Counties.
