WellBeam and Atrium Health Medical Group, part of Advocate Health, have launched a programme to advance care coordination between acute and post-acute providers through electronic medical record (EMR)-integrated interoperability.
Advocate Health also acts as a strategic investor in WellBeam, enabling more efficient clinical workflows and enhanced care coordination directly within users’ EMR systems.
The platform seeks to transform patient care across the continuum while reducing administrative workloads for clinical teams managing post-acute care.

WellBeam’s EMR-integrated platform will be introduced across several Atrium Health Medical Group sites, which form part of a comprehensive network of advanced practice providers, several physicians, and team members serving patients at more than 400 care locations.

WellBeam’s customer base includes health systems and independent medical groups with home health and hospice partners, citing up to a 25% reduction in emergency department visits from post-acute, and a 75% reduction in manual workflows per patient for physician teams overseeing post-acute care.
The Atrium Health Medical Group launch supports WellBeam’s vision to serve as a universal bridge between post-acute providers and hospitals, establishing connected, patient-centred care standards.
