AdventHealth has entered a collaboration with Biofourmis, a company specialising in technology-enabled care, to launch a remote patient management (RPM) programme in Central Florida, US.

This initiative aims to deliver whole-person care and an enhanced patient experience within the comfort of patients’ homes.

The partnership will see Biofourmis supporting the AdventHealth Hospital at Home programme by deploying its platform and remote clinical care team to facilitate inpatient-level care.

The enterprise platform is designed to provide complex, personalised care at home, featuring continuous monitoring and FDA-cleared artificial intelligence algorithms.

This technology will aid AdventHealth in expanding and scaling its care at home strategy.

The programme is set to be introduced at AdventHealth Winter Park.

The company is pioneering a standardised patient experience in Central Florida by offering post-acute RPM, which aims to keep patients engaged with their health through a consistent experience across hospital at home and post-discharge management. This approach ensures continuity of care.

AdventHealth Home Based Services Clinical Operations vice-president Maura Nazario said: “By bringing the same whole-person care found in our hospitals to your home, we can offer our patients a more comfortable and personalised healing experience increasing the patient’s autonomy.”

To enhance the programme, Biofourmis is integrating the health system’s Epic electronic health record (EHR) system to ensure a seamless integration of critical patient data.

The Biofourmis platform also includes a personal emergency response system, and arrhythmia and fall detection features, which provide safety assurances for patients and confidence for clinicians.

Additionally, AdventHealth will be able to schedule, coordinate, and monitor in-home ancillary services through the platform.
