Homewatch CareGivers, a provider of homecare for patients, has partnered with PreparedHealth, a Chicago-based healthcare technology company, to deliver improved care coordination in the home through latter’s enTouch.
enTouch is a digital network linking caregiver with patients from hospital to home.
Homewatch CareGivers president & CEO Julie R. Smith said: “We pride ourselves on pushing for growth and change within the industry.
“With more emphasis placed on home care by Medicare, we’re excited to provide innovative tools that can enhance patient care and allow us to build stronger relationships with our partners across the care continuum.”
enTouch connects all those involved in a patient’s Post-Acute Care (PAC), thereby allowing HIPAA-compliant and speedy life-saving communications.
Due to this technology, post-acute care providers are witnessing a growth in referral acceptance, quicker transition times and reduced readmissions in hospitals.
