US-based Mercy Medical Center-Des Moines has awarded TransformativeMed a multi-year contract to standardise and simplify electronic health record (HER) workflows.

As per the agreement, the not-for-profit hospital located in Des Moines, Iowa, will leverage TransformativeMed’s GlycemiCare platform to simplify EHR workflows for diabetes management.

Currently, Go-live is underway and GlycemiCare will be used by hospitalists and residents at the 875-bed Mercy Medical Center, which is a member of Mercy Health Network (MHN) and part of Catholic Health Initiatives (CHI).

GlycemiCare embeds in the Cerner EHR, thereby creating new clinical workflows that standardise and streamline the management and care of diabetes patients.

Clinical data, which is often scattered throughout the EHR, is organized and presented so that providers can quickly analyse trends and draw insights required to offer best-practice care and cut down unnecessary variability in glycemic control from admission to discharge.

In 2012, there were over 7.7 million hospital stays in the US for patients suffering with diabetes.
