US-based healthcare non-profit Montage Health has introduced Montage Health Connected Care, an at-home patient monitoring service powered by clinical software provider Cadence.

The new service is designed to improve the quality and accessibility of care for patients with chronic conditions.

It is initially focusing on hypertension, congestive heart failure and type 2 diabetes, but will later expand to include other conditions.

Montage Medical Group CEO Dr Mark Carvalho said: “This programme reduces emergency department trips because we are extending care into the patient’s home.

“It is a win-win for patients and clinicians. Our patients will have access from home to an expanded care team.

“Our clinicians gain better visibility into disease progression and benefit from the support of a nurse and practitioner-led care team helping to keep patients safe.”

The programme combines remote monitoring technology with a clinical care team led by nurse practitioners to monitor patients’ vitals in real-time, provide medication and coaching support and improve health outcomes.
