KishHealth System in DeKalb, Illinois, US, has been using the Pharmacy Xpert clinical surveillance and intervention system from Thomson Reuters to capture interventions and improve patient care.
KishHealth will implement Pharmacy Xpert in its Meditech hospital information system, which aggregates clinical data from patient admissions, pharmacy orders, laboratory results, vitals and various transcribed reporting systems.
Pharmacy Xpert, a clinical intelligence dashboard, is part of the Clinical Xpert suite of workflow solutions.
The system will monitor patient specific data from multiple sources, including medication lists and dosages, transcribed reports, vital signs, lab results, I&Os and demographics.
The patient-specific data, which is then delivered to the pharmacist on a single screen, is intended to allow proactive care management.
Pharmacy Xpert will offer prebuilt clinical profiles that deliver the drug, disease and toxicology content in the Micromedex evidence-based clinical decision support solution.
The prebuilt profiles include antimicrobial, anticoagulation and glycemic monitoring, IV to PO conversions, narrow therapeutic indexing, patient safety and target medications.
Pharmacy Xpert can be integrated within any hospital IT environment, including hospitals using Electronic Medical Record systems from Cerner, Eclipsys, Epic, GE, Meditech, McKesson and Siemens, the company said.
KishHealth System spokesman Chet Simmons said: "We anticipate capturing interventions that we might have overlooked previously by using the proactive alerts that Pharmacy Xpert offers."
"We see this as an opportunity to intercede earlier for patients who are at risk for adverse events, which we believe will result in increased patient safety as well as cost savings," Simmons said.
