US-based home health provider LHC Group has entered a new joint venture agreement with Baton Rouge General (BRG) Hospital.
The deal will improve home care service in Baton Rouge and help the seamless transition of patients from the hospital to the home.
Under the deal, LHC will serve as the preferred home health partner for all BRG patients. LHC will also create an office at the Mid City campus of BRG, a full-service community hospital with 590 beds on multiple campuses.
LHC Group chairman Keith Myers said: "LHC Group has a proven history of successful hospital partnerships that are enhancing post-acute care for patients around the nation, and we are excited about the opportunity to build on our record of success by working with the staff and patients of Baton Rouge General.
"Working as partners, we will deliver high-quality patient care, while helping patients recover and preventing costly and avoidable hospital readmissions.
"Baton Rouge General is recognised for its commitment to improving lives through the delivery of high-quality patient care, and a seamless continuum of care from hospital to home is part of that commitment."
In November, Baton Rouge General released its Future of Healthcare report, which outlined its plans to collaborate with other providers to improve community health and the convenience and affordability of care.
BRG president Mark Slyter said: "Our vision for the future includes continuing to seek out opportunities to collaborate with like-minded organisations. This agreement is one of those opportunities.
"We are proud to partner with LHC Group and their proven team of home health professionals.
"Working together, we can help more patients recover in the comfort of their own homes and avoid returning to the hospital unnecessarily."
More than 60 hospitals and health systems around the country partnered with LHC Group to deliver patient-centred care at home.
It is reported that patients with certain conditions are referred to home health as a part of recovery.
In order to help patients transition seamlessly into daily life, physicians will work closely with the home care team to deliver quality care.
Apart from providing health care services, home health professionals will also educate patients on their conditions, medication management and nutrition.
