US-based Post Acute Partners has agreed to acquire the McAuley Manor facility from St James Mercy Hospital (SJMH).
SJMH and Post Acute plans to execute an asset purchase agreement, following a period of due diligence.
St James Mercy Hospital president Jennifer Sullivan said: "Our new vision for healthcare in Hornell is designed to ensure continued access to quality health care services for our community.
"The selection of Post Acute Partners, a high-quality, highly reputable long-term care provider, is a very positive step toward achieving our mission."
With 120 beds, McAuley Manor provides long-term care services, and is said to expand Post Acute’s operations in the Hornell area. In May 2014, SJMH first announced its intention to sell McAuley Manor.
Post Acute Partners co-chief executive officer Warren Cole said: "We are excited about the opportunity to continue McAuley Manor’s proud legacy of service to the Hornell community.

Both parties need to obtain approval from New York State Department of Health for the deal. Post Acute, through Elderwood, owns and operates 17 skilled nursing, assisted living, and independent living facilities in New York.
The firm also provides services for six paediatric special care facilities in Pennsylvania, through Paediatric Specialty Care.
