US-based healthcare network NorthShore – Edward-Elmhurst Health (NS-EEH) has partnered with Lumeris, a technology-powered care enablement company, to enhance coordinated care and reduce healthcare costs.

The collaboration will integrate Lumeris’ population health data platform into NS-EEH’s clinically integrated network, which includes more than 3,000 physicians and nine hospitals in the Chicago metropolitan area.

It supports healthcare providers in care management, pharmacy management and patient engagement.

The partners will also focus on reducing healthcare disparities in underserved communities by creating new accountable care organisation (ACO) models.

They initially plan to participate in the Centers for Medicare & Medicaid Services’ ACO REACH model, which aims to improve care coordination and health outcomes for traditional Medicare patients.

In the future, NS-EEH and Lumeris intend to expand their focus to include other types of accountable care and population health models.
