Novant Health, in partnership with Health Recovery Solutions, has announced the launch of a remote patient monitoring system to advance care delivery and enhance outcomes.
A pilot programme to be launched by the partnership, will focus on bariatric patients.
Later this year, the programme is planned to be expanded to other patient populations.
With the monitoring system in place, patients will have the option to accept ongoing or follow-up care related to their condition from their home.
This approach is aimed at preventing hospital readmissions and enhancing patient outcomes.
Patients, who are enrolled in the programme, will be given a 4G Samsung tablet and Bluetooth biometric monitoring devices for recording their vital signs.

Through the programme, patients become more informed about their conditions and involved in their care.

“Using remote patient monitoring, we can now more closely monitor our patients in their own homes and, in real time, virtually track relevant physiological data such as weight, oxygen saturation and blood pressure.
“This degree of monitoring and patient involvement allows for an unprecedented level of care and has the ability to greatly improve patient engagement, education, support and overall outcomes.”
Novant Health clinicians will observe patients daily, answering risk alerts built on their reported vitals and symptoms.
