Indian technology services company Persistent Systems, in collaboration with Microsoft, has introduced a new generative artificial intelligence (AI)-driven population health management (PHM) solution.

This solution is designed to improve patient care and optimise healthcare providers’ capacity and cost-effectiveness.

The PHM Solution, which is in line with value-based care models, uses Social Determinants of Health (SDoH) to better predict healthcare costs and identify non-clinical patient needs.

A key feature of this solution is predictive risk modelling, which helps in identifying patients in need of significant medical support.

Persistent’s PHM Solution leverages Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights to extract SDoH from electronic health records (EHR) data.

According to Persistent Systems, approximately 80% of information available within EHRs is in the form of unstructured clinical notes and its PHM solution can accurately extract key patient data, unlike its counterparts in the market, which depend on third-party data sources or survey data.

The company said that the solution’s generative AI approach enhances patient outcomes by providing timely and intelligent care recommendations, offering higher accuracy and increased compliance compared to traditional natural language processing methods.

It also helps in recommending personalised interventions to reduce emergency visits, readmissions, and extended hospital stays, with a decrease of up to 60% in 30-day readmissions.

Persistent HLS Business Global Lead senior vice-president and general manager Ganesh Nathella said: “With this innovative solution, built on Persistent’s deep domain expertise and differentiated frameworks, along with Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights, we are identifying care access and health equity issues by optimising enterprise capacities and ensuring cost-effectiveness.

“We’re confident that through actionable and trackable insights and personalised intervention, this solution will significantly improve physician’s and caregiver’s decision-making to enhance patient experience and overall care quality.”

Persistent and Microsoft have a 32-year partnership, with Persistent having over 4,500 Microsoft-certified consultants.

The PHM Solution is now available on the Azure Marketplace.
