US-based healthcare technology company UTMHealthcare has collaborated with virtual care platform Somml Health to launch NuLink Health, a platform designed to enhance the care of patients with complex chronic ailments.

The new service seeks to improve patient outcomes and operational efficiencies for medical practices by reducing hospital readmissions for at-risk patients.

NuLink Health uses UTMHealthcare’s remote patient monitoring (RPM) solution and Somml Health’s virtual care platform.

The solution allows patient data to be shared securely in real-time across clinical settings, improving care coordination and provider ease.

The system data is fully merged into electronic medical records to ensure that up-to-date patient information is accessible to healthcare providers on real-time basis.

NuLink Health primarily targets patients with congestive heart failure, chronic obstructive pulmonary disease, diabetes, behavioural health issues, hypertension and chronic infections.
