Vytalize Health is a risk-bearing provider enablement platform, which addresses significant challenges in the healthcare industry with its care delivery system.

Through the latest partnership, Vytalize Health will use WellSky’s Next Generation Provider solution and services suite to improve care coordination and patient outcomes by supporting bi-directional communication between acute and post-acute providers.
Leveraging the suite, Vytalize Health also aims to improve sharing of critical patient information across care settings, ensuring that patients are discharged to high-performing post-acute facilities.

The partnership, which is already operational in several markets, also aims to enhance communication and collaboration between physicians.
It provides critical insights into a patient’s care journey, enabling providers to close care gaps, reduce unnecessary services and testing, prevent readmissions, and track patient locations and care timelines.
Vytalize Health chief product officer Frank Bordonaro said: “By partnering with WellSky, we can ensure these connections happen in real-time, such as when patients experience critical care events like a visit to the emergency room, which is a key to success in value-based care.”
