FAIR Health collaborates with NYC sites to enhance older adults’ health

The programme fosters shared decision making between caregivers, patients and providers, emphasising care decisions.

FAIR Health has partnered with three age-friendly clinical sites in New York City (NYC), US, to advance healthcare engagement for older adults.

The collaborations are part of the FH Sharing Healthy Aging Resources and Education (SHARE) NYC initiative, which is supported by funding from The New York Community Trust and involves Ryan Health, NYC Health + Hospitals/Bellevue, and Jamaica Hospital Medical Center.

As part of this initiative, these healthcare providers are using FAIR Health for Older Adults for engaging the patients and family caregivers at the point of care with shared decision-making tools.

The FH SHARE NYC programme is designed to foster decision making between caregivers, patients and providers, emphasising outcomes about care and treatment by balancing clinical evidence with patient preferences.

It broadens the use of FAIR Health’s shared decision-making tools at age-friendly sites.

Additionally, the initiative builds on lessons from other point of care efforts funded by the John A Hartford Foundation, aiming to gain unique insights from NYC’s diverse older adult population.

In addition to on-site clinical adoption, FAIR Health is sharing its resources and tools with local community organisations and social service providers throughout the New York metropolitan area.

The adaptability of FH SHARE NYC allows clinicians to choose how best to integrate these tools within existing workflows.

The condition-specific shared decision-making tools available on FAIR Health for Older Adults integrate clinical care and cost data.

Also available on FAIR Health for Older Adults are FH Total Treatment Cost tools for chronic and acute care, a Healthy Aging toolkit with articles and checklists, a dedicated section on dementia, and various patient and caregiver resources.

The New York Community Trust Programs and Grants vice-president Irfan Hasan said: “The New York Community Trust is proud to support FAIR Health's continuing efforts to provide older adults with the tools and resources they need to make informed healthcare decisions.”
