US-based health system Intermountain Health has formed a strategic partnership with Sensorum Health to enhance primary care access and health outcomes for older adults living with multiple chronic conditions.  

As part of this collaboration, Intermountain will integrate its expertise in clinical quality and patient-centred care with Sensorum's advanced remote monitoring technology. 

Sensorum Health's clinical team will work alongside Intermountain Health's staff to identify emergent health issues and create tailored clinical interventions.  

Through these efforts, the partnership aims to prevent unnecessary hospitalisations for patients by detecting early signs of chronic disease exacerbations.  

Sensorum Health CEO and co-founder Andrew Hotchkiss said: “Our solution integrates novel remote monitoring technology with specially trained clinical staff to identify the earliest signs of emerging health issues and triage patients to their primary care doctors for treatment.” 

Under the initial pilot initiative, the companies will also focus on homebound patients with conditions such as chronic obstructive pulmonary disease and congestive heart failure, enrolled in Intermountain’s population health entity Castell Health Salt Lake Valley "House Calls" programme. 

This pilot project is part of Intermountain's commitment to improving patient health outcomes through innovative care delivery methods. 

The results of the pilot project will be shared in whitepapers scheduled for publication in late 2024. 

Sensorum Health, supported by investors such as GV, Brighton Health Partners, and Operator Partners, specialises in home-based sensor technology and integrated clinical services.  

Intermountain Health tele-hospitalist programme and Castell home services medical director and internal medicine physician Dr Nathan Starr said: “Many patients prefer to be at home instead of the hospital. Sensorum offers a novel way to identify patients who are declining or getting sicker, often even before the patient realises it.  

“This enables us to follow up with the appropriate level of care and keep it with a central care team, before a patient's condition deteriorates and they need to seek emergency care. Sensorum does this without requiring older adults to adopt unfamiliar technology or disrupt their routine.”
