Tango has teamed up with WellSky to launch a comprehensive post-acute care management programme aimed at improving medical outcomes while reducing the total care cost.
The collaboration brings together Tango’s value-based home health enablement platform and networks with WellSky’s acute and post-acute technology, analytics, and services.

This is expected to address marketplace challenges such as limited or delayed access to optimised transitions within post-acute care, lack of effective integration and connectivity, disjointed incentives, and reimbursement issues.
